RELEASE OF LIABILITY

1. Voluntary Participation. I am voluntarily participating in martial arts, boxing, kickboxing, Muay Thai, jiu-jitsu, fitness training, sparring, conditioning, and other physical activities (the “Activities”) offered by True Warrior Fitness at the above location and any related premises, online, or off-site events.

2. Assumption of Risk. I understand that the Activities are inherently dangerous and involve risk of serious bodily injury, permanent disability, paralysis, and death, including risks from contact, strikes, throws, falls, grappling, use of equipment, the conduct of other participants, and the condition of the premises. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of those released below, and assume full responsibility for my participation. Initials:

3. Physical Condition. I affirm that I am physically fit and have no medical condition that would prevent safe participation. I assume all risk and responsibility for participating without a physician’s clearance.

4. Release and Waiver. To the fullest extent permitted by North Carolina law, I hereby release, waive, discharge, and covenant not to sue True Warrior Fitness, its owner Toby Grear, its trainers, instructors, coaches, employees, agents, volunteers, independent contractors, and the property owner and landlord of the premises (collectively, the “Released Parties”) from any and all claims, demands, losses, liabilities, or causes of action of any kind arising out of or related to my participation in the Activities, including those caused by the ordinary negligence of the Released Parties, whether to my person or property. 5. Indemnification. I agree to indemnify, defend, and hold harmless the Released Parties from any loss, liability, damage, or cost, including attorney’s fees, they may incur arising from my participation, whether caused by my own conduct or otherwise.

6. Medical Authorization. In the event of injury, I authorize the Released Parties to arrange emergency medical treatment, and I agree to be responsible for all related costs.

7. Media Release. I grant permission for True Warrior Fitness to use photographs and video of me taken during Activities for promotional and marketing purposes without compensation.

8. Governing Law & Severability. This Agreement is governed by the laws of the State of North Carolina. If any provision is held invalid, the remaining provisions remain in full force and effect.

9. Acknowledgment. I have read this entire Agreement, fully understand its terms, understand that I am giving up substantial rights including the right to sue, and sign it freely and voluntarily without inducement. Initials:

I am the parent or legal guardian of the above minor. I have read and agree to this Agreement on the minor’s behalf, consent to their participation, and agree to all of its terms, including the release and indemnification provisions, on behalf of myself and the minor.
